How to use your FSA/HSA for The Foundations of Wellness Program

A Step-by-Step Guide for New Clients

The Foundations of Wellness Program is a structured health behavior curriculum led by Andi Galusha, RN (CA), HWNC-BC. To utilize your pre-tax funds, please follow this process:

1. Schedule Your Complimentary Wellness Consultation (ZOOM)

Our journey begins with a video consultation (20–30 minutes). This face-to-face meeting ensures the program is an appropriate fit for your health goals. During this call, I will provide you with my Provider Information Sheet and a Letter of Medical Necessity (LoMN) template.

2. Connect with Your Healthcare Provider

Share the Provider Information Sheet and LoMN template with your primary healthcare provider (MD, NP, or PA). You can use this simple script:

"I am planning to work with a Board-Certified Health and Wellness Nurse Coach on a structured program to support my [e.g., blood sugar, heart health, or stress management]. Since this is a condition-specific program, would you be willing to sign this Letter of Medical Necessity so I can use my FSA/HSA funds for enrollment? Here is an information sheet for your review."

3. Finalize Your Enrollment & Submit

Once your provider has reviewed the materials and agrees that the program is appropriate for your health goals, follow these specific steps:

  • Check the Letter Before You Leave: Ensure your provider has signed the Letter of Medical Necessity (LoMN) and included a specific ICD-10 Diagnosis Code (e.g., codes for Hypertension, Prediabetes, or High Cholesterol). This code is what tells your insurance administrator that the program is a medical necessity rather than a general lifestyle choice.

  • Secure Your Documents: Keep the signed original LoMN in a safe place. You do not need to give it to me; you will need to upload it to your FSA/HSA portal or save it for your tax records.

  • Complete Enrollment: Use your FSA/HSA debit card (or your preferred payment method) to enroll in the Foundations of Wellness Program.

  • Download Your Itemized Receipt: Immediately following payment, I will provide you with a professional, itemized receipt. This receipt includes my credentials and the specific program curriculum details required by plan administrators.

  • Submit Your Claim: Log in to your FSA/HSA provider’s website or app. Upload both the signed LoMN and your itemized receipt to request reimbursement.

    Reminder: While the Foundations of Wellness is a qualified program, reimbursement is determined by your individual plan. We recommend confirming specific requirements with your administrator prior to enrollment.

Frequently Asked Questions & Financial Policies

  • Once you submit your itemized receipt and signed Letter of Medical Necessity (LoMN) to your plan administrator, the review process typically takes 2 to 6 weeks. This timeline is determined entirely by your specific insurance or plan provider.

  • No. Because The Foundations of Wellness is a structured, pre-approved curriculum, we begin our scheduled sessions as soon as your enrollment is complete. We do not wait for your plan administrator’s reimbursement cycle, as this is an independent financial process between you and your provider.

  • While I provide professional documentation and a pre-categorized FSA/HSA receipt, reimbursement is not guaranteed. If your claim is denied, you remain responsible for the full cost of the program. I recommend contacting your plan administrator before enrollment if you have concerns about your specific coverage.

  • Yes. My checkout system is categorized to accept most FSA and HSA debit cards at the time of enrollment. If the transaction is declined, it is usually due to a daily spend limit or a plan-specific restriction. In these cases, you can choose to pay with a standard card and submit your receipt for manual reimbursement.

  • A signed LoMN is a requirement for most FSA/HSA reimbursements. If your provider is unwilling to sign the form, you may still participate in the program as a "Private Pay" client, but you will likely not be able to use your pre-tax funds. We discuss this during your Complimentary Wellness Consultation to ensure you have a clear path forward.

  • If you are able to do so, I would recommend you scan all three documents (LoMN, receipt, and Provider Information Sheet) into a single PDF.

    Many insurance portals only allow one "attachment" per claim. If they upload just the receipt, it may get denied. If they have all three documents, you have a much better chance of getting approved.
